Home Assistant's built-in Nightscout integration shows your latest glucose reading and trend on a dashboard. Home Assistant is a home automation server. It reads an existing Nightscout site, so Libre and Dexcom feeds must reach that site first.
You can use its sensor in your own light or notification automations. A successful response can still hold an old reading, so check the timestamp before acting on a value.
It isn't a medical device. Nightscout readings can lag or fail, so keep your sensor's own app and alerts running.
